What is Surrogacy?

The dictionary meaning of the term `surrogate’ is a substitute.

Surrogacy – is an arrangement whereby a woman agrees to become pregnant for the purpose of gestating and giving birth to a child for others to raise.

The surrogate carries the genetic child of both the husband and wife through IVF/ICSI technique which requires the doctor’s intervention.

Ovum and sperm from the couple are removed and combined to create embryos in a lab. Then these embryos are transferred into the uterus of the surrogate.

The surrogate bears a child for a childless couple.

Who might benefit?

Research has stated that one-in-six couples have problems with infertility. Some use medical treatments to overcome this situation, for some however, no help is available.

It’s these couples where the woman has one/multiple reasons where she cant carry her own child this could be due to the following reasons:

Abnormalities in her uterus
Undergone a hysterectomy
Suffered multiple miscarriages
Born without a womb where surrogacy comes to the rescue.
Suffers from diseases like kidney disease, or similar other indications where carrying pregnancy can be detrimental to the woman’s life.
If she is on some cancer/lifesaving drugs which prevent her from carrying pregnancy.
Health condition that means it is dangerous for her to be pregnant or give birth.

Getting started

Let us help you making your baby. We will describe the whole process highlighting what is expected from you and your partner, how do we go about the surrogate preparation and what will be our role in the whole program.

Genetic Parents: Knowing, what is expected out of you, how to start and when.

Assessment	:	Please forward your and your partner’s detailed history, investigation reports, surgeries performed, any previous treatment taken, etc. (Assessment form)
Screening	:	You need to under go a specific tests to prepare yourself for the program (Genetic couple screening tests)
Surrogate Selection	:	It is totally your prerogative in choosing the surrogate for your baby. Our associate partner will advertise in the local newspaper for surrogate requirement on your behalf. Your assessment, screening and selection process will roughly take about 2-5 weeks.
Down Regulation	:	Along with the surrogate, you too are started on birth control pills to coordinate the cycles. GnRH analogues will be added in the later half (day 18) of the birth control pills and will continue till one day prior to ovum pick up.
Stimulation	:	You will be started on hormonal treatment (Inj. Gonadotropin) to stimulate your ovaries lasting 11-13 days under medical supervision.
Monitoring	:	You need to undergo baseline sonography and blood test to confirm the down regulation. Ultrasound guided follicular monitoring will be done by our doctor team.
Semen Freezing	:	A semen sample of your partner will be taken (to be frozen) as a back up.
Ovum Pick-up	:	Around day 13, ultrasound guided ovum pick will be done under general anaesthesia. You need to get admitted in the hospital for one day.
Semen Sample	:	On the same day, your partner needs to provide the semen sample, as fresh sperms always give better fertilization rate.
Fertilisation / Embryology	:	IVF / ICSI procedure will be performed in the lab. Fertilisation is checked the next day. The embryos thus formed are incubated further, till the embryo transfer into the surrogate on day 3 / 5.

Profiles	:	Surrogates profiles are prepared and sent to you for quick reference.
Screening	:	The surrogate will be screened (surrogate screening tests) for basic blood examination, infectious diseases, pap smear, genetic karyotyping, hysteroscopy, etc.
Counseling	:	The surrogate will be evaluated for her emotional and mental readiness by our Psychiatric team. This will be followed by the surrogate and her husband’s counseling enabling them to be familiar with the complete surrogacy program.
Down Regulation	:	The surrogate will be started on birth control pills to coordinate with your cycles. This synchronizes to obtain mature eggs from you and be transferred into a perfectly prepared endometrium to maximize the pregnancy success rate.
Endometrial preparation	:	The surrogate will be started on medications to prepare her endometrium for embryo transfer with natural estrogen and progesterone. Because each woman is a little different; the dose, duration and method of administering these hormones may need to be individualized. This would take approximately 12-14 days.
Embryo transfer	:	The embryos are allowed to grow in the lab for 2-3 days to make sure they are healthy and dividing properly for somewhere between 4-8 celled stages. Transfer will be done to the surrogate on day 2/3 post ovum pick-ups.
Progesterone support	:	This support is started for the surrogate prior to the day of embryo transfer and continued for the next 14 days.
Pregnancy testing	:	Serum B-HCG levels are tested 14 days later to confirm the pregnancy. If surrogate’s pregnancy test is positive, it will be repeated in 48 hours, and possibly repeated a third time in another 48 hours. This is to help assess the quality of the pregnancy.
Medications	:	We ask the surrogate to continue taking prenatal vitamins and Extra Folic Acid (4mg) everyday along with the progesterone support.

More recently, sophisticated assisted conception treatments have made it possible for a surrogate to carry a baby that is completely the biological child of the infertile couple. This is achieved by using an embryo conceived through in vitro fertilization (IVF) or a similar method, and is known as partial, gestational, or host IVF surrogacy. It is also possible for a surrogate to carry a donated embryo to term.

Surrogate arrangements are usually set up through agencies, although private arrangements exist. The couple is typically heavily involved in the pregnancy and the surrogate will remain a part of the baby’s life after birth. In both situations, the infertile couple usually pays the surrogate mother’s expenses along with additional legal and agency fees if a contract is involved. Expenses can include travel, maternity clothes, loss of earnings, medicines, USG and delivery expenses.
