Intra-Uterine Insemination (IUI)
During natural intercourse, only a fraction of the sperms deposited in the vagina make it to the woman’s fallopian tubes.
Assisted insemination (AI) or IUI is fertility enhancing procedure in which sperm are washed, concentrated and injected directly into a woman’s uterus through the cervix with a thin tube (a catheter) at the right time (When chances of fertilization are maximum).
Assisted insemination increases the number of sperm in the uterus and fallopian tubes where fertilization takes place and selects the most active sperm-cells to do so and also remove the negative compounds from the semen sample by processing it.
AI is most successful when it is used along with fertility drugs to enhance ovulation. There are a number of different fertility drugs that are available. Some of these drugs can be taken orally; others need to be given by injection.
The costs of these drugs, success rates and side effects are very variable and specific. This technique is called ovulation induction and AI or IUI.
It is the first step in treating the couples with unexplained infertility. It is cheaper, simpler and less invasive than other ART procedures. Conception is allowed to occur naturally after IUI.
Indications for IUI
Who is the suitable candidate for IUI treatment? In which cases would IUI be recommended and useful? Mentioned below are few indications for IUI.
• Unexplained Infertility
• Male Infertility – Mild Oligozoospermia, Asthenozoospermia, or Teratozoospermia.
• Failure to conceive after OI treatment.
• Immunological (Antisperm Antibodies)
• Ejaculatory failures
• Retrograde ejaculation
• Cervical factor

The Insemination Procedure
The inseminations are scheduled at the time of ovulation. This procedure is relatively simple and takes a few minutes to perform. The semen sample is injected into the uterine cavity through a special IUI catheter, thus passing the cervix so that an increased number of sperms can reach the uterine cavity and subsequently the fallopian tubes where the fertilization occurs.
Success Rate & Risk
The success rates of ovulation induction and intrauterine insemination depend on a number of factors. Of the various factors that determine the success rate of IUI program, we consider that the diagnosis and female partner’s age play a significant role in the pregnancy outcome. If the female partner is older than 35 years, chances of successful pregnancy are significantly decreased. Quality of the male partner sperm count is also one of the most important factors. The main risks that the fertility drugs that are used to stimulate the ovaries increase the risk of multiple pregnancy and rarely ovarian hyper stimulation syndrome.
